  • Human relevance of the rodent findings has not been determined. There have been no controlled studies establishing a causal relationship between GLP-1 receptor agonists and MTC in humans.

Contraindications

These medications are contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Patient Counseling

Counsel patients regarding the potential risk of MTC and inform them of the symptoms of thyroid tumors — including a mass or lump in the neck, difficulty swallowing (dysphagia), shortness of breath (dyspnea), and persistent hoarseness. Routine monitoring of serum calcitonin or thyroid ultrasound is of uncertain value for early detection of MTC in patients treated with these medications.

This boxed warning is the FDA's most stringent warning for prescription drugs. It applies to all GLP-1 receptor agonists including semaglutide and tirzepatide. Discuss any concerns with your Surfbound provider during the intake review.
