## Three dated findings

1. Current-source snapshot — September 19, 2026 (denominator: 8 offer rows). The dataset recorded 8 advertised offers
   across 5 providers. That is 8 offer rows, not 8 providers, and it is not a complete census of the market. Some fields
   were not reverified in the checked text and are labeled "Not verified" or "Not reverified" on the entry they belong to.

2. Historical membership fees — August 26, 2026 dataset (denominator: 15 provider entries). Of the 15 provider entries
   recorded, 8 of 15 recorded a separate membership or program fee in addition to an advertised medication price. This
   count describes the dated August 26, 2026 dataset only. It is not a current market fact.

3. Historical dose-based pricing — August 26, 2026 dataset (denominator: 15 provider entries). In the same 15-entry
   dataset, 4 of 15 entries explicitly recorded dose-based pricing, where the advertised monthly price rises as the dose
   increases. Two other entries explicitly recorded flat pricing across doses; the remaining entries varied by plan or
   could not be confirmed.

## How to read a quote: medication-only vs. membership-inclusive

A "medication from" price is often medication only — it can exclude a separate membership or program fee, and sometimes
excludes diagnostics, shipping or consultation. Those are frequently billed as a separate charge. When comparing
providers, ask whether the number you are looking at includes membership, or whether membership is added on top. A single
"all-inclusive" price and a "medication + membership" price are different structures, even when the headline numbers look
similar. This index shows medication price and membership fee as separate line items for that reason; it does not sum them
into one universal total, because the true total depends on dose, state and what each program includes.

## How to read a quote: prepay vs. monthly

A prepaid supply and a month-to-month plan can carry the same supply but a different amount charged today. A 3-month or
6-month prepaid supply is a single purchase of medication for that period, and it is not the same as billing of exactly 90
or 180 days. Plans may also advertise an introductory rate that later rises, or a lower per-month rate that is only
available with a longer prepayment. When comparing, match the same medication, the same supply term and the same payment
schedule before comparing totals.
